just like to say that your sig is the best sig I have ever seen, I dislike matt Hughes and I would wear that sig for a long time.

It's basically the user title. But admins can put HTML in the user title and so you can include an image rather than text.
I don't remember exactly, but I think the mods decided that if you had a certain amount of rep, 1,000 or 2,000 you could request to have one added. But they have to be relatively small. Ideally smaller than your av. (Unless you're Bullock, lol).
